Routine Appointments

All surgeries are by appointment with the Doctor or Nurse of your choice, if available, and booked at a minimum of 10 minute intervals.

Please note that when you book an appointment you will be asked a series of questions, these questions will help us in the following ways;

  • To try and ensure we are able to give you an appropriate appointment for your needs
  • From the most appropriate health professional
  • At the most appropriate time

Please be reassured that your personal information and health matters are always treated in the strictest confidence by all members of staff.

Thank you

Appointments at Chirk may be made by calling at the surgery or by phoning reception on 01691 772 434 from 8.00 a.m.- 6.30 p.m. We have several incoming lines so please be patient if your call is not answered immediately.

On-line Appointment Booking System

From 1st December 2015, patients can book routine appointments to see a GP at Chirk through our on-line booking facility.  Please ask at reception for a registration form which provides all the information you will need to use the My Health Online internet facility.  You will need to bring some form of ID with you (photographic) before you can be issued with a password and pin number.  If you have already registered for repeat prescriptions on My Health Online, you are automatically registered for the appointment booking facility which can be accessed by clicking on the 'Book your Appointment' icon on the homepage.  You can also cancel appointments using the on-line facility.

St Martins

Appointments at St Martins may be made by calling at the surgery or by phoning reception on 01691 773 334 during opening hours. If St Martins is closed your call will be automatically transferred to Chirk; Receptionists there will be able to make an appointment for you at St Martins.

If you cannot keep an appointment please let us know so that it can be offered to someone else.

Urgent Appointments

Please telephone the surgery and make it clear to the Receptionist that you consider your case is urgent. We currently run a GP triage system for urgent appointments and home visits. When you call a receptionist will ask for details of your problem and your telephone number. A doctor will then call you back and agree a course of action with you. This may result in an appointment a home visit or advice.

Home Visits

Please telephone Reception before 11.00 a.m. if you require a visit at home because you are too unwell to come to a surgery. Please give the Receptionist as much information as possible in order to help the doctors to allocate the priority of house calls. Please see above for details of the triage system. Lack of transport is not in itself a reason for a home visit.

Please remember that several patients can be seen in surgery in the time that it takes to make one house call.

One doctor is on call each day for emergencies in addition to his/her normal work - please only call if you really cannot wait.

Out Of Hours Calls

Out of Hours medical care is now the responsibility of the Betsi Cadwaladr University Health Board who have contracted the service to the North Wales GP Out of Hours (OOH) Service. When the Surgery is closed you should ring

0300 123 55 66

You will be answered by a trained call handler who will take basic details from you and arrange for your needs to be assessed by a health professional. You will be given advice and may be asked to attend a primary care treatment centre or receive a home visit if it is necessary.

N.B. If a patient's life seems at risk call an ambulance (999) as well as North Wales GP Out of Hours Service.

Minor Surgery

Doctors perform minor surgery at Chirk Hospital each day of the week. When you see your doctor he/she will ask you to contact the hospital in order to make an appointment.
